Your flight is canceled by Hawaiian Airlines. -
You decline to be rebooked on another flight despite the fact that your flight's departure or arrival time will be delayed by more than two hours. -
Hawaiian Airlines agrees to reimburse your fare if you or a member of your family is admitted to the hospital (after you complete a Medical Waiver Request form and send a scanned copy to HawaiianAirlines.com/CAO).
